What's Happening in the Brain Behind OCPD

It's tempting to think of personality traits like perfectionism or rigidity as simply ‘the way someone is’ — but there's a growing body of research looking at what's happening in the brain behind Obsessive-Compulsive Personality Disorder (OCPD). A 2022 systematic review pulled together the neurobiological research on OCPD for the first time.

The researchers found consistent evidence of impaired ‘cognitive flexibility’ — essentially, difficulty switching mental gears or adapting when a plan needs to change — along with executive function differences that show up on neuropsychological testing, not just self-report. Brain imaging studies pointed to involvement of regions including the precuneus and the amygdala, which is interesting because it suggests OCPD may involve somewhat different brain circuitry to OCD, even though the two conditions are often confused because of their similar names.

Importantly, the review was honest about its limits: the authors concluded there still isn't enough data to build a complete picture of what's happening neurologically in OCPD, and called for more research.

Why share this? Because understanding that rigidity and perfectionism have a real neurobiological basis — not just a matter of willpower or stubbornness — can be genuinely relieving. It's a pattern the brain has learned, and patterns that are learned can also be worked with in therapy.
